Incident Reporting is essential to follow up properly on concerns or issues that may arise.
This form can be used for medical/injury issues, safety issues, grievence issue or other as determined.
To make this process effective it should be submitted in writing within a minimum of 48 hours after the incident of concern.

You should recieve notice of reception by that party within 24 hours of that person receiving, (weekends/holidays are an exception)
