Coaching Application and Background Check

All persons interested in being either a head coach or an assistant coach must review and enter the following Forms:

• P.L.A.Y. Background Check Information
• Coaching Application
• Background Check Form

To become a Coach for the P.L.A.Y. Sport of your choice you will need to submit the Coach Application and Background Check Form for each season. This is a precaution for both you and the athletes, and greatly helps the Coordinators when completed on time. Your commitment is important to the growth of the program. You may be required to attend a Coaches clinic/meeting as well. Make sure both Forms are completed and signed. If you do not have experience, the clinics will be a benefit to you. The Director of the level of each sport will determine the coaches based on past evaluations, experience and needs.

Coaching forms are due in by the following dates:
